I can see your point on the limb thing.
My thought as far as time being equal was you cover the location and it 
heals what it touches.  Like with The Healing Favor and Toxic Favor, and 
Lay On Hands Miracle, I'd think no time listed means no extra time to heal.
I don't know about a Medicine roll per se, but if they have the skill 
Medicine: General they would be used to applying topical ointment.  Thus, 
if they had that skill, I'd allow it even sans Witchcraft for application.
