Depends on what you take as canon or not.  I know there have been a few 
times where he had though the Joker had been killed, but he keeps comming 
back.

If he has killed anyone, its was most likley the classic accidental fall 
from a high place death.  Where he actually might have tried to save them 
from falling but they still died.
