I'd say no, it's not cumulative.  Once you are ugly enogh for people to
avoid looking straight at you any extra ugly doesn't make it any worse.  It
might be worth giving hima  terror rating though, since he may (depending
on the mutations) look more like a humanoid monster than a mutated human.

Her's another question I don't think is covered in HoE: are mutation passed
on to children?  If so are the same mutations passed on, or do the children
get randomly determined mutations, or do the children get the same chance
of mutating at a child born to ordinary parents?.
