you know, you could say a Medince tn5(?) roll is required to extract the 
bullet.
Unextracted bullets give a penatly on the Vigor check to heal natuarally.
Removing them after the wound is healed causes a light wound to the 
location.

One could complicate the game a lot with such rules.  Wasn't it El Dorado 
where the Duke had a bullet lodged near his spine that was giving him 
trouble?
