Boom! I love that sound.
The malfunction tables are in the core DL rulebook. Every time a device blows
its Reliability, or Stability in your Junker conversion, it has a chance of
going <sproing>.
You roll 2D6.
The results vary depending on the device but 2-5 gets you a Major Malfunction,
6-10 is a Minor Malfunction, and the dreaded 11-12 is a Catastrophe. Or a Big
explosion usually.
The bell curve holds things in the minor malfunction section usually and the
results of each is determined by the type of device being used. The Marshal's
section has various examples that would help round the possibilities down. Again
this is in the DL core books.
