They are either templars who left the order, or individuals trained by such
a person. They gain a Greater Reward by killing a templar, but I don't
believe there is a correlation between the Templar they kill and the Greater
Reward they get as a result. They are basically a loose association of
individuals - at least, there's been no real suggestion that they are
banding together in some manner.

They become megalomaniacal as a result of that taint, due to the influence
of gun spirits.

They still exist, although there doesn't seem to be many outside of the
Sioux Nation.

Four of the five "pollution" spirit types are polluted derivatives of the
four elements (Radiation = Fire, Sludge = Water, Smog = Air, Trash =
Garbage). The fifth "type" are Insect spirits, which are a little
confusing, IMO - they embrace anyone specializing in any of the four "forms"
of pollution, as long as it kills humans.
Toxic Shamans work substantially different than the Shamans in Ghost
Dancers. They have a Strain-based spirit rather then worry about
Appeasement points. They are probably closest in form to Doomsayers from
the current HoE sets then anything, in that they pay Strain, and how they
recover that strain is dependent on the environment.
Hope that helps.
