Actually, I started off my campaign in Iowa. One problem is that it seems to 
be a pretty much wide open area (as is Missouri and Minnesota except for 
Mall of America).

Another problem (although also somewhat of an advantage) is that it is 
pretty far away from what seem to be the "centers of action" for HoE to date 
(Las Vegas, Denver, San Diego/Librarians, Boise/Templars).

I didn't really treat Iowa any differently.  Yes, Famine was through there.  
But so was War (heading east towards the Mississippi after his/its battles 
with the Sioux).  I figure the NA made its last stand against War's hordes 
there, and there are a lot of battlefields (and Walkin' Dead).

In my setting, Iowa City was the HQ of a offspring Aztec cult run by some 
renegade Wendigo sykers.  It didn't get GR bombed.

Cedar Rapids took a hit.  Des Moines and Quad Cities took two.

Keokuk (SE Iowa on the Missouri border) has been transformed into a "perfect 
little village"/Disneyland-type setting (kinda like the underground in Boy 
and His Dog) where the folks greet visitors with choruses of "Be Our Guest", 
feed them, clothe them, bed them, let them pass over to Missouri through the 
Mason-Dixon Wall, and...

...do nothing.  Creeped the party out to no end, but it wasn't like the 
locals were cannibals or anything.  They had bought some potatoes filled 
with that virus, but the party made their (secret) Vigor checks.  The 
200-yr. old Huckster tried a Hunch on the venison they were serving and 
backlashed, picking up a major phobia about eating meat, but other than 
that, not much happened there.  Of course, if the PCs ever return there it 
may be a different story.
