Make him afraid of blood - seen so much of it and it never seems to wash
off....

Or a fear of being afraid - or rather of being found out that he's afraid.
Being in that kind of conflict, the PC has come to believe that he's an
utter coward, but he can't let anybody know it - after all, his buddies
stood there in the breach, and how could he do less.  Or maybe the PC
survived by an act of cowardice, while the rest of his squad died for it.
Maybe one of his old buddies is still whispering in his ear.... "You left
us to die pal......we almost made it anyway, maybe we coulda made it if we
had a bit more firepower there...." 
Maybe it's just a purely psychological manifestation of his guilt, maybe....
