Umm. I'm going by the thing on the bottom of page 201.

"Black Joker

If this card was chosen during character creation, choose any Deuce as the
mutation. It's bad, but you get some choice. If the card's drawn curing play,
the mutation's fatal. Your survivor now isn't. He might still come back as a
Harrowed, though."
