Given that Call of Cthulhu (non-D20 version) is one of my other favorite 
games, I'd have to say that for a player who has only 1D6 in Guts...  that's 
too bad.  In a game like Deadlands or Call of Cthulhu, having a low Guts or 
Sanity respectively is going to carry penalties.  Short of retiring such a 
character, perhaps throw some more severe insanities at the character.  
Bearing in mind, not all play groups may be comfortable with them.

A few possible examples I've used in other games of insanities induced by 
horror:

Amnesia

Hallucinatory Phobia (you said he has a lot of phobias...  with this 
development, the character begins to envision the subject of the phobia(s) 
even when they are not really there)

Tics, tremors, or other disorders that inhibit/prevent certain activities 
(such as speech or writing, shooting straight, or whatever)

Psychosomatic loss of sight, hearing, or the use of one or more limbs

Strange or deviant habits (can relate to eating, compulsive rituals like 
checking one's gun constantly or walking in a strange pattern, or if your 
game is up to handling it even some form of sexual deviancy)

Echopraxia or echolalia, which basically means that the character mimics the 
actions or speech of someone else.  A great way for this one to be played is 
where the insane character perceives someone as stronger than they and thus 
better able to cope...  so they start repeating everything that person does 
or says.

Basically, just keep stepping up the level of the insanity, either adding to 
or possibly replacing existing derangements with more severe ones.  Games 
like Call of Cthulhu and some of the Palladium books, or White Wolf's 
Vampire, all have good lists of insanities.

Are you marshals using the scart table from marshals handbook?...  In the 
group I'm GM'ing at the moment there is actually one character with 1d6 in 
guts. Needless to say he fumbles alot, and I'm running out of good ideas on 
what to do with him (he's on the brink on diggin' himself a hole and stay 
there though, so that's working out as it should, but still).
