some dogs aren't 'fixable'.  If this dog is unstable and a danger 
to you and other humans and dogs, then you really do have some tough 
decisions to make. The best decision is an educated one and you are 
posting to the list and seeking help from behaviorists and trainers, 
but in the end it is your decision and you must make it. You do have 
options, live with the restrictions this dog places on your 
lifestyle, put the dog down, continue with training & behaviour 
modification, return the dog to rescue with it's full history 
available.  Whatever you decide, consider it a learning experience.
