It was supposedly used in the end of the adventure contained in City O Sin.
If it wasn't used in your campaign you could always use it for major
dramatic effect. For instance you could use it on the Reckoners. Imagine the
Scene:
Insert into the first narrative on page 37:
Fleeing before the monstrous horde are the bloodied remnants of the Iron
Alliance. If ever there was a time for a ghost rock bomb, this would be it,
and apparently someone with a working radio agrees with you. Through the
tiny window of the APC you are barely make out something streaking across
the sky from the direction of Junkyard. It lands at the feet of the Horsemen
and their monstrous army exploding with the sound of a million souls
screaming in agony and despair, the sickly green ash slowly forming into the
shape of a giant skull. The sound tears through you like thousands of knives
ripping through your very soul, yet somehow, you remain conscious. As you
look through the haze you begin to wish you hadn't survived as you begin to
make out the shape of four gigantic figures. With a wave of a bony hand, the
abomination army rises once again and continues forward.
(continue with story as normal with a slightly more freaked out posse)

I'm not Shane so I don't know about the rest.
