It's a rifle.

No, the 10mm rounds used by the M-22 are rifle rounds with a long cartridge, 
while the standard 10mm rounds are stubby pistol cartridges.

to appear like a normal backpack, or would it be an obvious tipoff to a 
casual observer that the wearer is a  Cyborg?

Yeah, it can be disguised to pass casual inspection. Similar packs were used 
by power armor troops and some heavy weapon crews, so a pack of this nature 
doesn't automatically scream cyborg.
 
 3. The book suggests that nobody could possibly mistake a heavy can for 
anything other than  a cyborg.. so what would a heavy with NO cyber limb 
replacements look like? :)

The doctors/technicians involved in creating heavy cans were more concerned 
with functionality than aesthetics. Heavy cans have obvious surgical scars, 
odd bulges in strange places, uncovered diagnostic ports, and even the 
occasional visible LED readout. 
 
 4. I'm pretty sure the answer is "yes", but should the Hold-Out rail gun use 
the same overpenetration rules as the rail gun listed in Wasted West? >>

No. The shorter barrel doesn't accelerate the round to anywhere near the 
velocity of the XM-60 rifle.
