[HoE] production costs

I did time in a print shop in my youth, and you're right on the money when
you say it's production costs. When you're doing a book layout, you often
print many pages on one big sheet of paper, fold the big sheet down, then
trim all the sides and glue the cover on with the binding.
128 pages is probably 8 big sheets of 16 pages each (or 16 sheets of 8
pages, or some variation of lowest common denominators). This may be an
upper limit because more pages would make the cover bigger (the spine would
need to be wider), which could significantly increase the cost of the book.
Printing is a balance of maximizing efficiency and minimizing waste.
