Well if you dealing with the first addition.  It not in the main book but in 
the Wierd West Players guide(can't remember the exact name)

In Second edition, 1/2 of it is in the PHB, the other half is secret marshal 
only stuff so its in the MHB.

But for the basics, it gives you 15 free points to spend.  But your marshal 
then draws a card on a secret table, which could cause a whole lot of bad 
things to happen to you(everything from missing a limb to being terminally 
ill, plus some even worse things)
