How bout if the arm goes thought degeneration level 
as per the Harrowed disadvantage and is a carrier of 
a disease. Then each full moon it turn into a 
werewolf arm which allows it to heal up normal, the 
player get a super arm 3 nights out of 20, but is 
stuck with a disease ridden stinking undeadish arm 
the rest of the time.

or have him get the were arm on the full moon and 
have the area of his body get a bit larger each time.
