Let me put a plug in for Nightmares of Mine, from Chaosium.  ISBN 1-55806-367-6

It's an incredible book with general advice for GMs on how to RUN a horror 
campaign.  A fair amount is just common sense, but a lot of it is good 
advice on how to use NPCs, timing, setting, etc.
